Vital Access Care Foundation (VACF) is proud to partner with CalOptima Health to provide Community Health Worker (CHW) services to eligible members.

​

CHWs are individuals who understand the culture, language, and needs of the community. This program helps connect individuals with medical care, mental health services, insurance, food, housing, and other social support, for those who have Medi-Cal (also known as Medicaid).

Health Education​

• Provide easy-to-understand information about medical, physical, and mental health

• Offer workshops or one-on-one sessions on nutrition, exercise, stress management, and wellness

Health Navigation

• Schedule health-related appointments

• Listen and help connect individuals to health professionals

• Assist with Medi-Cal, CalFresh, and other social services

​

Individual Support or Advocacy

• Help individuals understand their rights and options to ensure their health needs are met

• Assist with filling out forms and connecting to needed services

​

Screening and Assessment

• Help identify health and social needs

• Support in addressing barriers to care​

PROVIDER REFERRALS

We partner with you to provide health education, promotion, and resource navigation/care coordination that complements your care plan and support healthy and happy individuals.
